Dear Supervisor,
You've known me as a reliable worker for the last 4 years. Lately, I have not been well especially the past few weeks since the weather got colder. I have been feeling really tired like having the flu but without any cold symptoms. I'll be having a complete work-up done - blood tests and TSH tests for my Thyroid. I want to overcome this health slump. I'll be filing my official leave today, so please I hope you could get my leave request approved as soon as possible. So wish me well and I hope I get better.

Sample Letter for Request for Leave
Dear [Name],
As you may have heard, I am getting married on the [date]. You are invited to our wedding. With this in mind, I am requesting a thirty (30) day leave of absence.
My fiance and I will now fulfill our lifelong dream and honeymoon in a cruise to the Carribean.
I will be back at my desk, with my full attention to the department, as always, on the [date]. My tasks and clientele are reassign to [name of employee] during this period. You may coordinate with her on matters regarding our department.
