Dear Sir,

I would like to request an extension of leave for another period of two weeks April 2-April 16, 2012. I would like to avail of all of my remaining vacation and sick leaves of which I have accumulated the last three years.

My lumbar surgery went well unfortunately, my period of recovery will extend beyond my previous leave request. I will be undergoing a series of therapies to ensure that my back will be in good shape post-surgery. Attached are my medical documents and doctor's order for your review.
