Employee Review Letter

Dear Supervisor,

My employee review is coming up.
Perhaps this is a good time to talk about
the results of that last training exam, and
more so, anything I could improve on?

I'm making a conscious effort to meet my productivity goals
before the month ends.

Hope we could set a time for a meeting
