Excuse Letter for Respiratory Problem

Dear Supervisor,

I'm taking leave today. I'm experiencing tightness in my chest, and quite feeling short of breath. I have this cough you see for over a week now and I'm on bronchodilators. But the wheezing hasn't stop, so I'm going for another check-up with a specialist today. I'll be back for work tomorrow.
