We are giving out SSS E-1 (Personal Record) forms at the College Guidance and Counseling Center. There are only limited copies, thus, the forms are only available on a first-come-first-served basis. The form can also be downloaded from the SSS website as shown on the SSS Number Application 101 video.

Bring your duly accomplished SSS E-1 forms, together with the complete requirements, on the Job Fair. See announcements for details.

Request Letter for UMID Card SSS ID

Still not receiving your UMID Card SSS ID on time? Here's a sample follow up letter you can send the SSS office.

Dear Sir,

I would like to follow my UMID Card SSS ID 03-848555226-5 filed last year March 26, 2015 at SSS Antipolo. I was advised that I would receive my UMID Card at my home address in 3 months. It has been more than six months already, I would like to follow up on the status of my UMID Card as I need this for other official transactions.

Status of SSS Loan Letter

Dear SSS Loan Officer,

I am in need of your assistance in following up the status of my SSS loan. I have applied for the salary loan last August 10,2016 and it was encoded in same date August 10,2016. When I checked online via my SSS account - it states that I already have a check date, a check number, check amount, and it also says "CHECK GENERATED". Expecting that this has been duly processed, I've waited 3 weeks already and I have yet to receive my check and I really need it badly for my family.

I hope someone from your end can clarify and assist me on this matter.

Follow-up Letter on SSS Salary Loan

To Whom It May Concern,

It's been three weeks that I still have not receive a cheque for my SSS salary loan. I went to the local post office and I was advised that it has not arrive. As I could remember it will only takes 2 weeks to process. Please do advise me what steps to take to finally avail of my approved loan cheque.

Request Letter for Disability Benefits

Here's how to correct your records when applying for Disability Benefits from your government. Availing of disability support can be tricky, so make sure your employment certificates are in order, also proof that you have been paying the security deposits to the government.

Dear Sir/ Madam:

May I request for a manual verification and computation of my actual Credited Years of Service (CYS) for the following reasons:

1.    Your records show that my actual Credited Years of Service (CYS) is just 22 years;

2.    My own records show,  as follows:

a. COMPANY A.-- from 1972-1973  -  5 years

b. COMPANY B -- from 1974-1998   -   20 years

Total Years of Service- 25 years

I enclosed herewith the documents needed to support my request.

a.  Certification of my actual years of service from COMPANY A
b.  Certification of my actual years of service from COMPANY B

Please make the necessary manual verification and computation to rectify my actual records and if proven to be in order, please make the necessary adjustment needed.

Please be reminded also that I have availed three (3) partial disability benefits, to wit;                                             

             1.   June 2003 to April 2005
             2.   July 2005 to Sept. 2006
             3.   Oct. 2006 to Aug. 2008

In connection with these, I believe there must be an adjustment of the benefits I received per availment based on your final analysis and findings.
