Excuse Letter for Work Because of Surgery
Dear HR Manager,
I will not be at work today and will most likely miss a few more days. I have spent the last five hours at the Emergency ward of Saint Claire's Hospital. I'll be undergoing an emergency endoscopy procedure, the doctor thinks I have ruptured something in my abdomen. My wife will be coming over at the office today to clarify some coverage issues with our health card medical provider. I will be requiring further hospitalization. Hope you can extend her some assistance with the paperwork.

Inquiry Letter SSS Change of Name to Married Status

I'm making a personal inquiry for my SSS account #1235463. I have a question regarding changing the name of my beneficiary- can I do this online? Also I will be changing my status and name since I recently got married - is that something that needs to be done in person? I am currently out of the country. I badly need a new ID with my married name on it, if so, what's the process for people working overseas?

Letter for DFA Passport Appointment - Request for Confirmation Code for DFA Appearance
Dear Sir,
I completed my online appointment application on the DFA Philippines website today July 26, so I can have an appointment on September 2 2016 in DFA Aseana. However, I have no received any confirmation email or Appointment number. I have checked my spam and junk mail as well. As much as I want to cancel the previous application the website says I should input my appointment code. As I do not have the confirmation or reference number because there is no DFA email to confirm this - I cannot do anything about my passport application status. I have called the hotline numerous times, both DFA and APO Productions, but there seems to be no one taking in any calls. Is there any official DFA advisory on the matter? My employer has booked my flight for October and it is important that I not lose my work contract. I just really need the appointment code so I can still make in time for my flight.
