Maternity Leave Letter to Boss

Dear Sir,

Though I have formally filed for my maternity leave, I would still like to inform you that I would be gone starting this Monday. I know you are quite concern about the regional accounts under my responsibility, please be assured that I have made the necessary preparations that the accounts are well cared for by my assistant Sandra while I am on leave. If you need any specific files, contact numbers and progress reports, she will accomodate your requests.

Planning your Maternity Leave: Sample Maternity Leave Letter to Boss

A detailed personal Maternity leave letter outlining Vacation Leave and Work concerns.

Dear Sir,

I'm now currently on my 24th week of pregnancy and I've decided that I'll continue working up to my 33rd week. My initial plan is to continue working on the projects assigned to me until the temp staffer arrives.

As early as now, it's really difficult for me to do site inspections and facilitate the training programs. I don't want to go on leave early but right now, my baby's safety is more important to me. I'm planning to take my leave of absence on the week of October 12, I will have about 15 days of leave plus another set of 15 days of vacation leaves I was unable to use the past year. Since the Vacation leave days are not convertible to cash or even possibly carried over to the next employment year, I have decided to use up all my VLs along with the allowed maternity leave days this year.

I am letting you know of my maternity leave plans in detail and as early as now; as Human Resources requires me to have your signature on my allowable number of VLs. Also I hope that the temp staffer or intern will be here by next week, so I can begin mentoring her. While I am away, you may reach me anytime via email or through my mobile number for anything work-related.

On this note, I would like to personally thank you and the company for the show of support and understanding throughout my pregnancy.

Sample Maternity Leave Letter Adding Vacation Leaves to the Leave Period

Maximize your Maternity leave by adding your remaining Vacation leaves (VLs) to the overall vacation period. The extra days will help you spend more time with your child. 

Dear HR Officer,

Please accept my formal notice of maternity leave. I am due in four months time, on 16 May 2011, please anticipate my return to work on June 30, 2011. My maternity leave spans more than the alloted month, but I am availing all my remaining vacation leaves to add up to this leave period. I would need the extra days to care for my infant until my childcare provider arrives, I have attached my OBGYNE note for my leave notice and in line with company policy, I also wish to request for my maternity benefits in full.

Regarding my work, I have advised Clara Ross, our junior marketing executive to oversee all the accounts I handle in the next few months. Rest assured, I will complete all my tasks and clear the primary invoices before my leave date. Please do not hesitate to call me if you need anything during this transition.

Sample Maternity Leave Letter Using Available Number of Remaining Sick Leaves

This sample maternity leave letter expresses the request of the employee to use her available sick days as part of her paid maternity leave.

Dear Supervisor,

I am requesting a maternity leave to commence approximately June 1 to June 30, 2010.

I would like to use my remaining number of available sick leaves (5 days) for that portion of the maternity leave that my doctor considers me disabled.

Please advised me the soonest as I am scheduled to undergo a C section anytime next week.

Human Resources Notice on Employee's Maternity Leave

Dear [name of employee],

Congratulations and thank you for telling me about your pregnancy and the date that your baby is due. I am writing to you about your maternity leave and pay.

As we have discussed, you are eligible for [number of weeks] maternity leave. Company policy on maternity leave is as follows:

[Insert Maternity Leave Policy]

Given your chosen start date of [insert date], we anticipate your return to work on [date]. Your maternity pay will be the amount of [insert amount], this is a per day amount based on your monthly salary. You are also entitled to maternity benefits under the company's health plan.

If you want to change the starting date of your leave you must, if at all possible, inform me at least a month before your proposed new start date.

If you decide to return to work before [insert date leave ends], you must give me at a few days notice.

During your maternity leave, please make sure to keep in touch. [Name of employee] is filling in for some of your work tasks and he/she might need assistance or instruction from time to time.

If you decide not to return to work you must still give me proper notice. Your decision will not affect your entitlement to your severance pay and other employee benefits.

If you have any questions about any aspect of your maternity leave, please do not hesitate to get in touch with me. Congratulations to you and your family as you anticipate the arrival of your little one.
