Dear Sir,

I had to abruptly leave the office due to a family emergency. My mother has seriously taken ill and was taken to the hospital for surgery that day. Unfortunately, I wasn't able to inform my supervisor of my reason for suddenly leaving work. I am filing the proper leave forms today for my missed work days - Monday and Tuesday. I hope you may reconsider my day of absence as a leave.

My mother is currently in recovery and my sister has taken over caring for her. I will not be missing any work days this week.
